The Insurance we accept is Workers Comp and No-Fault. If you have any other insurance you will get a receipt and submit it yourself.
They will reimburse you directly if it is a covered expense. 

Your sessions may be tax deductible if you have a doctors note recommending Massage Therapy. Please check with your accountant.

Before you set up an appointment for massage therapy, please provide us with the following information. Your information must be verified by my office before your first appointment.

• Your name, address, phone number
• Claim #
• Insurance Company phone number and contact person
• Name and address of Insurance company (where to send the bills)
• Date of injury
• A prescription for MASSAGE THERAPY. It must say massage therapy, not physical therapy or anything else. You can bring this when you come
  to your first treatment session.
• A valid credit card. This will only be charged if your insurance does not pay for the treatment. You can also bring this to your first session.
